Intellectual and Developmental Services Intake Process
To start the intake process, you will be asked to complete an intake packet.
You can pick up a packet at any of our Burke clinic locations or call Burke intake at 936-634-5010, and one can be faxed, emailed, or mailed to you. Once you have completed the packet, we ask you to return it to the Burke clinic near you with the other requested documents. If you can’t provide these, you can still have an appointment, and we will work with you to get the documents completed.
When you return the packet, Burke staff will meet with you to copy the documents you brought. Staff will then review and explain all the forms you completed. During this time, you will also complete a financial agreement that will explain any charges for Burke services. After this, your intake assessment will be set. Burke does intake in five different cities and also uses video conferencing called Lifesize if you want to do the intake from your home. You are free to choose the setting that works best for you. If you decide to use Lifesize, the staff will help you set it up. If you choose to use Lifesize, you must be alone in a quiet area when we call to start the assessment to protect your privacy. We normally set intake assessments the same day or the next day you turn in the packet, and we will set them at times that work for you.
On the day of your assessment, if you choose to come to a Burke clinic, you will check in at the front desk, and you will be seen by the next available clinician. If you choose to complete your assessment through Lifesize, you will be called by intake staff, who will help you get on it.
